We provide official readings for exams performed at facilities outside UCSF.
To utilize this service, please complete a requisition with the required information noted below, include the provider's signature, and submit along with the original images (CD or X-ray films) to the Radiology Imaging Library.
Required Requisition Information
- Patient name and medical record number
- Date and type of exam requested for second reading
- Signature of the referring physician and your telephone number
- Name, type of insurance company (e.g., Aetna PPO), and authorization, if necessary
