We’re located between 4th Street and 16th Street at 1700 4th Street, San Francisco, CA 94158. The building entrance is along Campus Way at the Genentech Hall main entrance.

Look for the large outdoor stone staircase. Once you arrive there, please call the front desk at (415) 514-4815, and we’ll meet you outside to escort you into the building.

Things to Know

Before your appointment

Complete the MRI screening form online via MyChart before your appointment.
Arrive 30 minutes before your scheduled time unless our nurse tells you otherwise.
Bring a photo ID for check-in.

Fragrance-free policy

The Surbeck Lab is a fragrance-free zone. Please avoid perfumes, colognes, and scented lotions to support staff and visitors who may be sensitive to scents.
